Medical Certificate in Mai Khao
Certificates are one of the few things people need from a doctor while feeling perfectly well. Airlines, insurers, employers, dive schools, visa offices. A medical certificate in Mai Khao can usually be issued during the visit — and what decides whether it is accepted is getting the right one in the first place.
What can be issued
The ones actually asked for by visitors here, in rough order of frequency:

Each has different content requirements and several have a specific form. Say which you need at the start of the visit rather than the end — the examination differs, and doing the wrong one means doing it twice.
Sick leave, and what an employer can reasonably ask
A sick note states that you were seen, on what date, and that you were unfit for work for a stated period. That is all it needs to say.
It does not need to name your diagnosis, and usually should not. If an employer is insisting on one, that is between you and them — what gets written is what is accurate and what you consent to, not whatever a third party demands.
Backdating is the one thing that will not happen. A certificate can record when symptoms began by your account, but it cannot claim you were examined on a day you were not. That is a false document, and an employer who checks will find out.
Insurance letters
The most valuable document most visitors here will ask for, and the one people request too late.
If illness costs you a tour, a transfer, extra nights or a changed flight, the insurer wants a doctor’s written statement that you were unfit to travel on those specific dates, produced at the time rather than reconstructed afterwards.

Fit to fly
Airlines ask in a narrower set of circumstances than people expect: recent surgery, a cast, later pregnancy, recent significant illness, or any visible injury that makes cabin crew uncertain.

The practical advice is the same either way: arrange it the day before rather than the morning of, because being fifteen minutes from the airport is exactly why people leave it too late.
Visa extensions and long-stay certificates
Thai immigration and the labour office both ask for a medical certificate for certain visa categories and for work permits, on a standard form covering a short list of conditions.
The wording and format matter more than the examination, and a letter on ordinary clinic paper is routinely rejected at the counter. If your agent, school or employer has given you a specific form, bring it — a form supplied by the people who will accept it is always safer than a generic one.
These usually carry a validity window, commonly thirty days from issue, so getting one months ahead is wasted effort. Time it to the appointment rather than to convenience.
Medication letters for travel
If you carry injectables, controlled drugs, sharps or a device such as an oxygen concentrator, a letter listing what you have and why is what security and customs at both ends will want.

Keep the letter separate from the medication itself, and keep both in hand luggage.
What will not be written
Nothing will be certified that has not been examined, nothing backdated, and no diagnosis stated that is not believed. A medical certificate is a legal document, and a false one is worth nothing to the person relying on it.
Fitness will not be certified where it is genuinely absent. If you are not fit to fly, dive or work, the certificate says so — which is the more useful answer, because the alternative is being turned away by somebody with more at stake than a form.
Within those limits it will be written clearly, in the format required, and you will be told up front if a document seems unlikely to be accepted.
What to bring
The specific form, if a school, employer, insurer or office has given you one. That single item prevents most rejections.
Your passport, since certificates are issued against passport name and number and a mismatch causes problems later. Any medication you take, and details of any relevant condition.
And the deadline. Most certificates are same-visit, but a few need a blood test first, and that changes the timing.
Getting one without the journey

Photograph the certificate before you hand it over to anybody. Originals get submitted to airlines, insurers and immigration offices and do not come back, and a photograph has repeatedly turned out to be the only surviving copy.
