Mai Khao backs onto mangrove, casuarina and the edge of Sirinat National Park, which is why it is quiet and green — and why the biting is worse here than on the busier beaches. Insect bite treatment in Mai Khao is mostly about stopping the itch and spotting the small number that become infected or mean something more.
What is biting you
Mosquitoes — the daytime ones matter most here, because they carry dengue. Evening and night mosquitoes are a nuisance rather than a risk in Phuket.
Sandflies, which are the specific problem on this coast. Tiny, almost invisible, and they bite around the ankles and feet at dawn and dusk. The bites are disproportionately itchy and last far longer than a mosquito bite.
Ants, including fire ants in the grass behind the beach, and occasionally bed bugs — a line or cluster of bites appearing overnight, in a pattern, is worth mentioning to your resort.
What actually helps the itch
Cool the area — a cold compress or a cool shower does more in the first hour than any cream.
Antihistamine tablets work better than antihistamine creams for widespread itching. A mild steroid cream is the most effective thing for individual angry bites.
Calamine is soothing and short-lived. What does not help: hot spoons, toothpaste, and scratching, which is the one that turns a bite into an infection.
Sandfly bites, and why they are worse
They deserve their own section because visitors are consistently caught out. The bite is often not felt at all, and the reaction appears hours later as intensely itchy red spots, usually below the knee.
They can itch for one to two weeks, far longer than a mosquito bite, and they frequently blister or leave dark marks that persist for months on paler skin.
They bite low and at dawn and dusk, so repellent on ankles and feet in the early morning and late afternoon prevents most of them. They are worst on the sand itself and in the dune grass.
When a bite has become infected
Bites in this climate infect readily, largely because people scratch them in their sleep. Watch for:
- Redness spreading outward beyond the bite, especially hour by hour
- Increasing pain after day two, when it should be settling
- Warmth, swelling and throbbing
- Yellow crusting, pus or discharge
- A red line tracking away from the bite
- Fever or feeling generally unwell

Severe allergic reactions
Most bite reactions are local, even the dramatic-looking ones. A whole forearm swelling after a bite is usually a large local reaction rather than a dangerous allergy.
What is dangerous: swelling of the lips, tongue or throat, difficulty breathing, a widespread rash away from the bite, dizziness or collapse. That is anaphylaxis and needs adrenaline immediately — call 1669.

When a bite means checking for something else
A fever three to seven days after being bitten in this region is worth taking seriously rather than attributing to a virus — that is the dengue window — see fever treatment.
A bite that is unusually painful, blistering badly or has a dark centre needs looking at rather than treating as ordinary.
And a bite you did not see happen, on a child, is worth asking about directly. Children rarely report them and frequently scratch them raw — see children and family care.
Repellent that works
DEET at 20 to 50 percent, or picaridin at 20 percent. Both are well evidenced. Higher DEET concentrations last longer rather than working better.
Apply after sunscreen, not before, and reapply after swimming or heavy sweating. Citronella and wristbands do not provide meaningful protection, whatever the packaging suggests.
For daytime dengue mosquitoes, morning application matters as much as evening. For sandflies, concentrate on ankles and feet at dawn and dusk — the two schedules overlap conveniently.
The resort environment
Mangrove and standing water nearby mean breeding sites close by, and heavy rain in the monsoon multiplies them within days.
Air conditioning genuinely helps, as do screens. Sleeping with a balcony door open onto a garden in Mai Khao is a reliable way to be bitten repeatedly.
Most resorts fog for mosquitoes periodically. If you are asthmatic, that is worth knowing about — the smoke can trigger symptoms and it is reasonable to ask when it is scheduled.
Bed bugs, and how to be sure
Worth separating from everything else, because the answer is a conversation with your resort rather than a cream.
The pattern is distinctive: bites appearing overnight in a line or a tight cluster, usually on skin that was uncovered in bed — arms, shoulders, neck, ankles. Mosquito bites are scattered and random; bed bug bites tend to follow a seam of contact.
Check the mattress seams and the headboard for small dark spots. If you find them, ask to change room rather than treating the bites and staying put, and keep your luggage off the bed and closed in the meantime — the bites themselves are harmless and itch like any other, but you will keep collecting new ones every night until the room changes.
Marks that outlast the holiday
